Data We Collect and Process

PrimeCare collects data from a variety of sources, including our websites, email communications, social media, and other platforms. We also collect medical information from our patients. How We Collect and Process Data

We might obtain data and personal information in a variety of ways:

• Information you voluntarily give us through a form or survey
• Cookies and similar tracking technologies
• Interactions with our website and social media profiles

How We Use and Share Data

If you are a patient of PrimeCare, we will not use your protected health information for marketing purposes without your consent. However, we may use and share certain data and information as we:

• Treat you
• Run our organization
• Bill for your services
• Help with public health and safety issues
• Do research
• Comply with the law
• Respond to organ and tissue donation requests
• Work with a medical examiner or funeral director
• Address workers' compensation, law enforcement, and other government requests
• Respond to lawsuits and legal actions

We will not share your information with third parties other than for the purposes listed above. We do not sell user data to third parties.

Ask us to limit what we use or share

• You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say "no" if it would affect your care.
• If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say "yes" unless a law requires us to share that information.

Get a list of those with whom we've shared information

• You can ask for a list (accounting) of the times we've shared your health information for six years prior to the date you ask, who we shared it with, and why.
• We will include all disclosures except for those about treatment, payment, and health care operations, and certain other disclosures. We'll provide one accounting per year for free but will charge a reasonable, cost-based fee if you ask for another within 12 months.

Other Data Privacy Rights

Depending on your country or U.S. state of residence, you may have additional data privacy rights. These rights might include:

• Accessing a copy of your data
• Requesting that we update or correct inaccurate data
• Opting in or out of certain types of data processing
• Requesting the deletion of your personal data

Your request must include:

• Information that reasonably verifies that you are the person about whom we collected personal data or their Authorized Agent

We cannot act on your request if you do not provide us with information that confirms your identity and the rights you wish to assert.

Individuals in the European Union

We do not knowingly collect personally identifiable information from individuals residing in the European Union or United Kingdom.
